About

National Action Plan for Childhood-onset Heart Disease (CoHD) – Educating, supporting and resourcing healthcare professionals to promote excellence in whole of life care for the Australian CoHD community.

CoHD includes all heart abnormalities affecting patients before and after birth during childhood, including acquired and congenital heart disease. There are an estimated 250,000 affected individuals with CoHD in Australia, with likely over 1 million people impacted when we consider their carers and family. The National Action Plan (AP) for CoHD, which is supported by Heartkids, is a multi-tiered approach to support integrated, quality health care for all stages of the CoHD care pathway. A key component of the AP is to create education, supportive resources and tools for healthcare professionals (HCPs) and the CoHD community. Resources are evidence based and co-designed with HCPs and the CoHD community in alignment with the Australian Standards of Care for CoHD. This talk will outline the wealth of resources available to you as a HCPs, the types of resources available, how to easily access the education and resources, and how you can participate in the creation of new resources and other education projects.

Key takeaway points from the presentation:

  • New Australian specific resources are available to help support HCPs in the care of patients with COHD and their families.
  • The resources are codesigned with health experts and the CoHD community and aligned with the Australian Standards of Care for COHD to create best practice evidence-based education.
  • Australia specific CoHD education for the health workforce is essential in standardising and providing quality healthcare.
  • HCPs can connect patients and their families with Australia specific CoHD community targeted resources, support services and educational events which have been developed as part of the AP.
  • Access to these national resources is free and available to all healthcare providers working with individuals and families with CoHD.
