Kathryn is a speech pathologist who has worked in acute speech pathology service for 15 years, most recently in the areas of neonatal and paediatric feeding, FEES, and tracheostomy management. In the last 3 years, Kathryn has held a joint role as senior speech pathologist and paediatric care navigator in the recently initiated Paediatric Liaison and Navigation Service (PLaNS) at Canberra Health Services. In this role, Kathryn works with a team of nurses and AHPs to help coordinate care for families and children with chronic and complex health needs who access interstate health care.

About

Care navigator positions exist in other health jurisdictions, however across Australia they have invariably been filled by registered nurses.  In 2023, Canberra Health Service was unique in utilising AHPs in these roles. Multidisciplinary care navigators – Nursing and Allied Health Professionals (AHPs) – work with families, local and interstate health providers, and community supports and services, to improve healthcare access, experiences, and outcomes for children with chronic and complex health care needs. This presentation will outline the development of care navigation roles for AHPs, as well as the organisational challenges and positive benefits for staff, services, organisations, and consumers.
